It's a MK38 Mod2 25mm gun, also known as the Typhoon weapons system, seen here mounted on USS Pearl Harbor.
It replaced the MK13 missile launcher.

Pontos has a wonderful detail up set that includes the platform and gun. You just need to make sure you order the correct one.
