I could see this happening 50 years ago or so, I just don't understand how it happens now. I thought our subs were chock full of sophisticated sonar and detection devices that should tell them any ships in their vicinity so when they surfaced they wouldn't get smacked. Having never been on a submarine, I don't know what is and what isn't though.
